Co-payments, Deductibles that may be required for your visits are due at the time of your visit. For your convenience we accept cash, personal checks and major credit cards.
If your plan requires a Referral, it is your responsibility to obtain one from your primary care physician. Our office will inform you if your referral is current and valid for each of your office visits.
